Sotyktu is a disease-controlling treatment: it calms the immune pathway driving psoriasis and psoriatic arthritis, relieving skin and joint symptoms and helping protect joints from damage. Improvement builds over 2–4 months — it is not a painkiller, and the daily tablet only works while it is taken consistently.

What it is

Sotyktu (deucravacitinib) is a once-daily pill in a newer class called TYK2 inhibitors. It calms one specific inflammation pathway inside cells — tyrosine kinase 2 — which helps drive psoriasis and psoriatic arthritis. Because it targets one narrow pathway rather than the immune system broadly, it works differently from both biologics and the older JAK inhibitor pills. It is used for adults with moderate-to-severe plaque psoriasis and for psoriatic arthritis, and it is an option many patients like because it is simply one tablet a day.

Before starting

Before starting, we screen for infections including tuberculosis (TB), because any medicine that calms immune pathways can allow a hidden infection to become active — finding and treating one first keeps you safe. We check basic blood work, and we may check triglycerides (a blood fat) and liver tests, because Sotyktu can raise triglyceride levels in some people and we want a baseline to compare against. Tell us about any muscle pain conditions, since a muscle enzyme test may be checked if symptoms occur. Sotyktu is not combined with other strong immune-suppressing medicines, and live vaccines should be avoided while taking it.

How it’s given

It is one tablet taken by mouth once a day, with or without food, swallowed whole — do not crush, cut, or chew it. Taking it at the same time each day helps keep the level steady. There are no starter doses and no injections. Store it at room temperature in its original container.

What to expect

Skin improvement usually builds over the first 2–4 months, with results judged around 4 months; joint symptoms in psoriatic arthritis improve over a similar period. The most common side effects are cold symptoms and upper respiratory infections, cold sores, acne-like rash or follicle inflammation, and mouth sores. Blood tests occasionally show a raised muscle enzyme (CK) or triglycerides, which is why we check them if symptoms or your history suggest it. Because immune pathways are being calmed, take infections seriously — call us for fever or an infection that is not improving.

Everyday guidance

If you miss a dose, take it when you remember that day; if it is nearly time for the next one, skip the missed dose — never double up. Keep taking Sotyktu even when your skin is clear: the inflammation returns when treatment stops. Tell other clinicians you take an immune-calming pill, and call us before starting new medicines, including supplements, so we can check for interactions. If you are planning a pregnancy, discuss timing with your provider. Questions: call the office at (361) 400-7700.

Common worries

Is this one of the JAK inhibitors I’ve seen warnings about?

No — Sotyktu targets a different, narrower pathway (TYK2) and was designed to avoid the broader JAK effects. It carries its own, smaller set of cautions, which we review with you, but it is a different class of medicine.

Why did I get cold sores / acne after starting?

Both are recognised, usually mild effects of this medicine — cold sores because immune signalling is being adjusted, and acne-like rashes from the skin’s response. Tell us if they are troublesome; they can usually be managed without stopping treatment.

No injections and no lab routine — is it really strong enough?

Yes — in trials it cleared skin to a degree comparable with several biologics. Strength comes from hitting the right pathway, not from the route. One tablet a day is the full treatment.
