- Self-injection at home
- How often
- Once a month
Bimzelx does both jobs: it relieves symptoms and, in psoriatic arthritis and spine disease, helps protect joints from ongoing damage. Skin improvement often begins within a month, with full effect judged at about 4 months. It is a maintenance medicine, not a painkiller — keep to the schedule even when you feel completely well.
What it is
Bimzelx (bimekizumab) is a biologic medicine that blocks two related proteins, interleukin-17A and interleukin-17F (IL-17A and IL-17F), which drive inflammation in the skin, joints, and spine. Blocking both can produce deep skin clearing in plaque psoriasis. It also treats psoriatic arthritis, ankylosing spondylitis, and non-radiographic axial spondyloarthritis — inflammation of the spine that does not yet show on regular X-rays. It is a maintenance medicine, taken on a regular schedule to keep inflammation controlled.
Before starting
Before starting, we screen for tuberculosis (TB) with a blood test, because immune-calming medicines can allow a hidden TB infection to become active — treating any inactive infection first makes long-term therapy safe. We also check liver blood tests before and during treatment, because Bimzelx can raise liver enzyme levels in some people; those numbers tell us the liver is handling the medicine well. Tell us if you have inflammatory bowel disease (Crohn’s or ulcerative colitis), as IL-17 blockers can aggravate it. Live vaccines are avoided during treatment, so completing vaccinations beforehand is ideal.
How it’s given
It is an injection under the skin that you can learn to give yourself at home with a pre-filled pen, into the thigh, belly, or upper arm. For psoriasis, injections are given every 4 weeks at first, and many patients then move to one dose every 8 weeks; for psoriatic arthritis and spine conditions, the usual schedule is one dose every 4 weeks. Each dose may be two injections. Store pens in the refrigerator, let one warm for about 30 minutes before injecting for comfort, and rotate injection sites.
What to expect
Skin improvement often begins within the first month, and results are usually judged at about 4 months; joint and back symptoms improve over 2–3 months. The most common side effects are cold symptoms and upper respiratory infections, headache, diarrhoea, tiredness, and soreness at the injection site. Yeast infections — particularly oral thrush (white patches in the mouth) — occur more often with this medicine than with some others, because IL-17 helps keep yeast in check; tell us if you notice mouth soreness or white patches, as they are easily treated. Call promptly for fever or any infection that is not settling.
Everyday guidance
If you miss a dose, take it as soon as possible and then return to your regular schedule. Keep taking Bimzelx even when your skin is clear — the benefit depends on steady treatment, and psoriasis returns when it is stopped. Keep your lab appointments even when you feel well: the blood tests are how we confirm your liver is tolerating the medicine. Tell other clinicians that you use an immune-calming biologic, and check with us about vaccines and surgery. Questions are always welcome at (361) 400-7700.
Common worries
I’ve read it can cause thrush. Should I worry?
Yeast infections — usually mild oral thrush (white patches in the mouth) — are more common with Bimzelx than with some biologics, because IL-17 helps keep yeast in check. They are straightforward to treat; just tell us if you notice mouth soreness or white patches.
Why do I need liver blood tests?
Bimzelx can raise liver enzyme numbers in some people without causing symptoms. Checking before and during treatment catches any change early, when adjusting treatment is simple.
Will the injections hurt much?
Most people find the pen quick and manageable. Warming it to room temperature for 30 minutes first and rotating sites are the two best tricks; our team will watch your first injection.
